This is an unconfirmed transport request submission. With space allowing, we will review your request(s) and send confirmation with an approved pick up and delivery location. If this is your first time using our service, you will need to get in touch with the coordinator before submitting this transport request form.

  • STOP. Both sending and receiving partners are required to be approved Rescue Express Rescue Partners prior to placement of a Transport Request. Confirm that your partner has an approved Rescue Partner Application and Rescue Partner Contract on file by contacting coordinator@rescueexpress.org or (541) 525-3232 before placing a request. Requests for unapproved partners will not be processed. Adopters are not required to become Rescue Partners. Please read the Rescue Express Partner Guidelines for information about transport procedures including: submitting transport requests, bus stop locations and schedules, loading/unloading procedures, reservation changes/cancellations, quarantine guidelines and more.
  • Where Rescue Express initially loads the animal(s). Location addresses will be included in your transport approval email. Submit separate transport requests if you are requesting transport to/from more than one loading or unloading location.
  • Please indicate whether the animal(s) are being delivered to a rescue organization or an adopter
  • Pickup Information

  • The shelter/rescue/individual/boarding facility located in the state the animal is being loaded in and caring for the animal(s) during the required 10-day quarantine period.
  • Address of the shelter/rescue/individual/boarding facility that is caring for the animal(s) during the required 10-day quarantine period. This CANNOT be the same as the receiving address.
  • First & Last Name of the person delivering the animal(s) to Rescue Express.
  • Person meeting the bus and loading the animal(s)
